Man hole cover

Saturday at approx 11:43 myself and my partner were walking the dog near kingsbury water park/ coton road when my partner fell down a man hole cover, when walking this man hole cover was not visible as it was covered in mud/grass which led to my partner walking over it and falling down leaving him with a swollen left thigh and in alot of pain. This also caused our dog who he was walking to fall into the busy Coton road.
As you can imagine I am extremely unhappy about this issue as walking around that area there are NO signs of man hole covers being unstable. I have attached photos of the man hole cover and you are able to see how this was left once my partner had fallen down. Today my partner has woke up with extreme back pain and hopefully will get a doctors appointment today to be checked out. Please can you get back to me ASAP on where I go from here.
I did contact somebody Saturday however i contacted the wrong people which is why I'm contacting you today. Thanks

THIS HAS BEEN REPORTED AGAINST THE WRONG COTON ROAD. COTON ROAD NUNEATON IS NO WHERE NEAR KINGSBURY WATER PARK.
