Uneven pathway (raised pavement)

Unsecure pavement and brick paving issue. My grandmother (88 years old) was walking through Union Walk that goes into Court Street Car Park after getting off the G1 bus. She tripped and fall over the uneven walk way on a loose brick, she ended up in hospital having to have stitches above her eyebrow and a deep bloody wound. She was covered in bruises from the fall and has lost confidence in herself where she was normally a fit an able lady of her age group otherwise. So aside from pointing out the uneven footpath that needs to be re-surfaced I would like to put in a claim for her injury. Please explain the process to me and I will submit accordingly. I have photos of her stitches and can supply them along with a hospital report as well as a witness statement from the meat shop opposite who assisted her prior to family arriving to assist and reassure her.
